If you have already purchased insurance, you do not need to fill out this form again.
The "Calculate Insurance Cover Needed" form will tell you how much insurance you will need to purchase for the dates you wish to be covered for. You can then pay for the amount of time you need cover for using the "Purchase Insurance Cover" form at the bottom of the page.
The minimum insurance fee is US $197 for 30 days. If you are purchasing more than 30 days of insurance, you will be charged for the additional days on a pro rata basis at $5 per day. The maximum insurance cover available using this form is 180 days.
The form below will calculate the price of your cover; as an indication here are some prices of different lengths of time:
- 30 days: US $197
- 60 days: US $347
- 90 days: US $497
The online payment form will add 5% to cover Worldpay's online charges.
